Appointment cancellations are to be made before date of appointment or full consultation fee will be levied/charged.
 
For all new appointments please bring the following:

• A current referral letter from your local doctor, specialist or hospital
• Valid Medicare Card, Private Health Insurance
• A summary of your current medications (prescription and over the counter and allergies history)
• Recent blood results
• Imaging results (x-rays, ultrasound scan, MRI, PET scan)
• Other clinical results (e.g. Endoscopy)
• Records of previous surgery/ injury (trauma)
• Immunisation history

• Clinical Review - $150
